What Causes Shoulder Pain?
The shoulder is not a single joint — it’s a complex of four joints working together, supported by muscles, tendons, ligaments, and bursae. Problems in any of these structures can cause pain, and identifying exactly what’s going on is essential before treatment begins. At Move Easy Osteopathy in Bristol, we commonly treat:
Rotator cuff injuries the rotator cuff is a group of four muscles and their tendons that stabilise the shoulder and control its movement. Tears, strains, and tendinopathy of the rotator cuff are among the most common causes of shoulder pain we see — particularly in active patients and those who do repetitive overhead work. Symptoms typically include pain at the front or side of the shoulder, weakness when lifting, and pain that wakes you at night.
Shoulder impingement syndrome also known as subacromial impingement, this occurs when the tendons of the rotator cuff become pinched between the bones of the shoulder during movement. It causes a characteristic painful arc — pain when lifting the arm between roughly 60 and 120 degrees. Impingement is very common and responds well to osteopathic treatment and rehabilitation.
Frozen shoulder (adhesive capsulitis) a condition in which the shoulder joint capsule becomes inflamed and progressively stiffens, severely restricting movement in all directions. Frozen shoulder typically progresses through three phases — freezing, frozen, and thawing — and can last 18 months to three years without treatment. Osteopathic treatment and guided rehabilitation can significantly reduce the duration and severity of each phase.
Acromioclavicular (AC) joint problems the AC joint, at the top of the shoulder, is commonly injured in falls and contact sports, and can also develop degenerative changes over time. AC joint pain is typically felt at the very top of the shoulder and worsens with cross-body movements.
Bicep tendinopathy inflammation or degeneration of the bicep tendon causes pain at the front of the shoulder, often worsening with lifting and overhead activity. Commonly mistaken for rotator cuff problems, accurate diagnosis is key.
Referred pain from the neck and upper back not all shoulder pain originates in the shoulder. The joints and muscles of the cervical spine and thoracic spine frequently refer pain into the shoulder and upper arm. Treating the neck and upper back is often a key part of resolving shoulder pain — which is why a whole-body assessment is essential.
Osteoarthritis degenerative changes in the shoulder joint are common in older patients and in those with a history of injury or heavy manual work. Osteopathic treatment cannot reverse degenerative changes but can significantly reduce pain, improve movement, and slow progression.
