What Is Sciatica?

Sciatica describes pain or altered sensation travelling along the path of the sciatic nerve. This runs from your lower back, through your buttock, and down one or both legs. It’s caused by irritation or compression of the sciatic nerve, usually in the lower spine.

It’s extremely common, but that doesn’t mean you have to put up with it.

What Causes Sciatica?

The most common causes we see in our Bristol clinic include:
  • Disc Herniation (“slipped disc”) the most frequent cause; a disc in the lower spine bulges and presses on the nerve root
  • Piriformis syndrome  an over-diagnosed cause. The piriformis muscle in the buttock irritates the sciatic nerve .
  • Lumbar spinal stenosis — narrowing of the spinal canal, more common in older patients
  • Degenerative disc disease — age-related changes that reduce disc height and increase nerve irritation
  • Spondylolisthesis — where one vertebra slips forward over another
  • Muscle tension and poor movement patterns — sometimes the cause is mechanical rather than structural
  • Sports and training injuries, overload, poor mechanics, or a specific incident during exercise or sports.
  • Pregnancy-related back pain, postural changes, joint laxity, and activity changes during pregnancy frequently cause irritation and inflammation around the nerve roots
  • Stress and tension. Chronic stress manifests physically and sometimes this can irritate the nerve.

Getting an accurate diagnosis matters enormously — piriformis syndrome and disc herniation require very different treatment approaches. This is where a thorough osteopathic assessment is invaluable.
